Protein plays a vital role in wound healing, yet its role is often underestimated or underprioritized in clinical practice. Regardless of the wound etiology, ensuring adequate protein intake is essential for supporting tissue repair and promoting optimal healing outcomes.

While advanced wound care products often receive the spotlight, optimal wound healing begins with a strong nutritional foundation. Without sufficient protein, the body simply lacks the building blocks needed to create new tissue, produce collagen, support immune function, and fight infection.

For wound care clinicians, protein assessment is a practical clinical skill. Knowing when needs increase and how to estimate an appropriate target helps support timely tissue repair, individualized nutrition planning, and better healing outcomes.

Why protein is essential for wound healing

Protein is made up of amino acids that serve as the structural components of nearly every tissue in the body. During wound healing, protein supports multiple physiological processes, including:

  • Collagen synthesis
  • Fibroblast proliferation
  • Angiogenesis (formation of new blood vessels)
  • Immune cell production and function
  • Enzyme and hormone production
  • Tissue remodeling
  • Maintenance of lean body mass

When protein intake is inadequate, the healing process can slow, and tissue repair may be compromised. Patients may experience delayed granulation tissue formation, decreased wound tensile strength, prolonged inflammatory responses, increased infection risk, and impaired immune function.

According to the Academy of Nutrition and Dietetics, Protein-energy malnutrition (PEM) is a state of inadequate protein and energy intake that results in loss of lean body mass, impaired immune function, and delayed wound healing. Protein-energy malnutrition is particularly common among older adults, hospitalized patients, individuals with chronic illness, and residents of long-term care facilities — all populations frequently encountered in wound care. Early identification of PEM is critical because inadequate nutrition can increase the risk of wound complications.

Increased protein requirements during healing

The Recommended Dietary Allowance  (RDA) for protein depends on the circumstances. For healthy adults, the minimum amount to meet the needs is 0.8 g protein/kg/day. This recommendation is designed to prevent deficiency rather than support wound healing.

Patients with wounds, however, often require substantially more protein due to increased metabolic demands and the need for tissue repair. According to the National Pressure Injury Advisory Panel (NPIAP), patients with pressure injuries should receive a comprehensive nutrition assessment and an individualized nutrition care plan. The guideline also recommends implementing protein supplementation for patients who are malnourished or at risk of malnutrition when nutritional needs cannot be met through food intake alone.

Although protein needs should always be individualized, wound care clinicians commonly use a clinical target of 1.25–1.5 g/kg/day for adults with pressure injuries and other chronic wounds when there are no medical contraindications, such as advanced kidney disease. Patients with severe burns, extensive tissue loss, or significant protein losses may require even higher amounts based on clinical judgment and consultation with a registered dietitian.

General protein recommendations include:

Healthy adults: 0.8 g/kg/day

Older adults without wounds: 1.0–1.2 g/kg/day

Patients with chronic wounds or pressure injuries: approximately 1.25–1.5 g/kg/day

Patients with severe wounds, burns, or extensive tissue loss: up to 2.0 g/kg/day, depending on clinical status and nutrition assessment.

How to calculate protein requirements

Calculating protein requirements is relatively straightforward using body weight measured in kilograms.

Step 1: Convert pounds to kilograms

Start by converting the patient’s body weight from pounds to kilograms using the following formula:

Weight in pounds ÷ 2.2 = weight in kilograms

Example: A patient who weighs 176 pounds weighs approximately 80 kg because 176 ÷ 2.2 = 80.

Step 2: Select an appropriate protein goal

Choose a protein target based on wound severity, nutrition status, and clinical judgment. For a patient with a Stage 3 pressure injury, a commonly used goal is:

1.25–1.5 g/kg/day

Step 3: Calculate the daily protein range

Multiply the patient’s weight in kilograms by the selected protein target. For an 80 kg patient, the calculation is:

  • 80 × 1.25 = 100 grams/day
  • 80 × 1.5 = 120 grams/day

The patient’s estimated protein requirement is 100–120 grams per day.

These calculations provide a practical starting point, but nutrition interventions should always be individualized based on the patient’s overall health status, laboratory findings, appetite, and medical conditions.

Should actual or ideal body weight be used?

This is one of the most common questions clinicians ask.

For most patients, actual body weight is used to estimate protein needs. However, calculations become more complicated in patients with obesity, edema, amputations, or significant fluid overload.

The NPIAP recommends that patients identified as being at nutritional risk receive a comprehensive nutrition assessment from a Registered Dietitian (RD). Registered dietitians are best equipped to determine the most appropriate method for estimating calorie and protein requirements in complex patients.

When uncertainty exists, collaboration with the RD is considered best practice.

Signs a patient may not be meeting protein needs

Protein deficiency can be difficult to recognize. Patients at higher nutritional risk may show signs such as:

  • Unintentional weight loss
  • Poor appetite
  • Loss of muscle mass
  • Weakness or fatigue
  • Delayed wound healing
  • Recurrent wound breakdown
  • Frequent infections
  • Low dietary intake

The NPIAP recommends routine nutrition screening for patients with pressure injuries and comprehensive nutrition assessments for those identified as being at risk of malnutrition.

Food sources of protein

Helping patients identify protein-rich foods can improve intake without relying solely on supplements.

Examples include:

  • Lean poultry
  • Fish
  • Eggs
  • Greek yogurt
  • Cottage cheese
  • Milk
  • Cheese
  • Beans
  • Lentils
  • Soy products
  • Nuts and nut butters
  • Seeds
  • Lean beef
  • Protein-fortified beverages

For patients who cannot meet their protein needs through diet alone, oral nutrition supplements may be appropriate. The American Society for Parenteral and Enteral Nutrition (ASPEN) provides evidence-based clinical guidance to support safe, individualized nutrition care for patients with complex nutrition needs. Consider the whole nutrition picture

While protein is essential, effective wound healing also requires adequate overall nutrition. Calories provide the energy needed to spare protein for tissue repair, while vitamins and minerals, including vitamin C, vitamin A, zinc, and copper, support collagen formation, immune function, and cellular repair.

Hydration is equally important. Even mild dehydration can impair circulation and reduce oxygen and nutrient delivery to healing tissues.

Nutrition is only one component of comprehensive wound management. Pressure redistribution, infection prevention, moisture management, vascular assessment, glycemic control, and treatment of the underlying cause of the wound all contribute to successful healing. The NPIAP emphasizes that nutrition should be integrated into the overall plan of care rather than addressed as a stand-alone intervention.

Protein calculations are only one piece of the puzzle

Calculating protein needs provides clinicians with an evidence-based starting point, but nutritional care should always be individualized. Factors such as chronic kidney disease, liver disease, dialysis, obesity, frailty, and severe illness may require modified recommendations.

Registered dietitians remain essential members of the interdisciplinary wound care team. They can perform comprehensive nutrition assessments, determine individualized calorie and protein goals, recommend appropriate supplementation, and monitor progress throughout the healing process.

By knowing how to estimate protein requirements and identify when additional nutrition support is needed, clinicians can better support wound healing and patient outcomes.
